‘Scots, England and ‘Britain’: Constitutional Niceties’

Over the summer much was made of the performance of 'Team GB' at the Olympics and Paralympics, while in 2014 a referendum on Scottish independence will take place. 

What does 'Britain' mean? 

Who shaped the use of the term after the Union of the Crowns in 1603 and of the Parliaments in 1707?

Does 'Britain' mean the same thing in England and in Scotland - and, if not, did it ever? 

In what way can the United Kingdom be called united, and how far is disunity built into its very constitutional structures? 

Why was Mary Queen of Scots - not of Scotland - and Elizabeth I Queen of England - not of the English?
